Client - Consultant Service Agreement
As a client of The Scrapbook Artist, I understand and agree to each of the following provisions:
1. I understand that only items labeled “acid free,” “lignin free,” and
“of archival quality,” will be used in the making of this album. All
products come from reputable companies. Any possible damage from
defective products is not the liability of The Scrapbook Artist or
Doreen M. Helm. I also understand that if I choose styles that use
“embellishments” that may not be of archival quality, these items may
damage my photos.
2. I understand that photos and memorabilia will be mounted using
permanent and archival safe adhesive, unless otherwise specified. I
also understand that removal of photos is not recommended, as it may
damage the photos. Any photos or memorabilia that are not to be mounted
permanently are noted on the inventory sheet. These items will be
adhered with photo corners.
3. I understand that it is my responsibility to ensure that the photos
provided to The Scrapbook Artist can be replaced. In the event that
irreplaceable photos are damaged, I accept full responsibility and will
not hold The Scrapbook Artist and or Doreen M. Helm liable. I also
understand that should the photos be damaged in the event of a natural
disaster or other unavoidable circumstance (theft, car accident, etc.),
it is my responsibility and I will not hold The Scrapbook Artist
accountable. In the event that the photos and or memorabilia are
destroyed via an unavoidable circumstance, The Scrapbook Artist will be
responsible for the cost of reprinting the photographs.
4. I understand that photos may be cut (“cropped”), and that any photos
I do not want cropped are indicated either on the inventory sheet or on
a label attached to the photo.
5. I understand that The Scrapbook Artist requires a
non-refundable deposit of 50% at the time this agreement is signed. I
also understand the remaining balance will be paid upon delivery of the
album. The Scrapbook Artist accepts the following forms of payment:

6. When the project is finished, I understand that I will be notified
and the remaining balance will be due at that time. I will provide the
balance in full when the project is delivered. If it is being shipped,
I will provide the balance due prior to shipping.
7. If the project is to be shipped, The Scrapbook Artist will ship it
through a reputable shipping agent. I understand that it will be
registered and insured, with proof of delivery required. I also
understand that The Scrapbook Artist is only responsible for the
shipping insurance. Any loss or damage that occurs will be the
liability of the shipping agent. Any shipping expenses incurred by The
Scrapbook Artist will be added to my account and included in the
“balance due.”
8. I understand that acceptance of the album is acceptance and satisfaction of all work completed.
9. I understand that I cannot return a completed project and get a refund.
10. I understand that The Scrapbook Artist will make every effort
possible to complete this album on the date indicated in this
agreement. If it is not ready by that date, The Scrapbook Artist will
give you a 10% discount on the total balance.
11. I understand that all returned checks will be subject to a $30
inconvenience fee and work on the project will stop until payment is
made. If the payment is not re-made within seven days, returned
checks will be forwarded to collection authorities.
12. I understand that the design work in my album is copyright The
Scrapbook Artist. It is not to be used in any magazine, contest, Web
site, or anywhere else unless my complete copyright is also
included. In addition, these unique designs may not be
scraplifted or photocopied.
13. If I agree to the use of computer titling and/or journaling in the
album, I understand that the computer ink used is labeled “light
resistant” up to [insert the number the ink manufacturer states] years.
Any possible fading or damage is not the liability of The Scrapbook
Artist or Doreen M. Helm.
14. Any photos or memorabilia not included in the album will be returned to me.
15. I agree not to alter the finished book in any way without the help
or permission of Doreen M. Helm or The Scrapbook Artist. I understand
that my album is the work of an artist and changing that work is
altering a piece of art. I also understand that altering or adding to
an album could cause immediate or future damage to my album and/or
photos.
16. I understand that I must use caution if and when removing pages
from the completed album (for intents such as journaling). I understand
that though paper and embellishments are secured well, they will not
withstand abuse or frequent removal from protectors. If any damage
occurs due to frequent removal, I understand that I will be charged a
separate fee for The Scrapbook Artist to repair the pages.
17. Upon confirming I have read and agree to the terms of this agreement, I will be presented with a scope
of services and related fee letter from The Scrapbook Artist, which
will outline my project in specifics and the related cost. If you agree to the terms of this Client-Consultant Agreement. Please close this window and complete the form.
